PAYMENT

You are responsible for paying at the time of your session unless prior arrangements have been made.  Payment must be made by your insurance company, EAP, check, cash, or a credit card.

SELF PAY

The self-pay rate for counseling is $130 for the first appointment if it goes beyond 60 minutes, $110 for each appointment afterwards (for 53-minute sessions), $80 for 45-minute appointments, and $60 for 30-minute appointments.  For a crisis session it is $110 for the first 53 minutes and then $60 for each additional 30 minutes.  Consultation sessions are $90 per hour.  These are paid by check, cash, or credit card.  Any checks returned to are subject to an additional fee of $25.00 to cover the bank fees.  If you refuse to pay your debt, I reserve the right to use an attorney or collection agency to secure payment.  In addition to weekly appointments, it is my practice to charge this amount on a prorated basis (I will break down the hourly cost based on self-pay rates) for other professional services that you may require such as report writing, telephone conversations that last longer than 15 minutes, attendance at meetings or consultations which you have requested, or the time required to perform any other service which you may request of me. (This does not apply for EAP clients).  Any fees associated with court involvement will be your responsibility at the rate of $200.00 an hour. If you cancel your appointment prior to 24 hours or no show, you will be subject to a $50 charge of the session. If you have insurance that we do not accept and choose to use the self-pay option, we can provide you with a superbill.  It is your responsibility to submit the superbill to your insurance company for reimbursement.  On a case-by-case basis we may submit a courtesy bill.

PARENTING SESSIONS

We highly encourage parenting sessions (family therapy without the client present), some insurances cover this, however there are some insurances that do not. The rate follows the same rate as a therapy session ($110 for each appointment afterwards (for 53-minute sessions), $80 for 45-minute appointments, and $60 for 30-minute appointments.)

UNIQUE SERVICES

Sometimes it may be clinically appropriate for a therapist to consult with providers outside a therapy appointment. Consulting with providers will follow the same pay rate as a therapy session (23 to 30 min-$60, 38 to 45 min- $80, 53 to 60 min- $110, over an hour $130 flat fee.) Most insurance companies do not cover this.

INSURANCE

If you are using commercial insurance your co-payment will be assessed with your insurance company.  If you have a health insurance policy, it will usually provide some coverage for mental health treatment.  You are responsible for knowing your coverage and for letting me know if/when your coverage changes.  You should also be aware that most insurance companies require you to authorize me to provide them with a clinical diagnosis.  By signing this Agreement, you agree that I can provide requested information to your carrier if you plan to pay with insurance.  ** Please be aware insurance companies have the rights to all your records and at times call providers to discuss the details of your treatment.  In addition, if you plan to use your insurance, authorization from the insurance company may be required before they will cover therapy fees.  If you did not obtain authorization and it is required, you may be responsible for full payment of the fee.  Many policies leave a percentage of the fee (which is called co-insurance) or a flat dollar amount (referred to as a co-payment) to be covered by the patient.  Either amount is to be paid at the time of the visit by check, credit, or cash. Some insurance companies also have a deductible, which is an out-of-pocket amount that must be paid by the patient before the insurance companies are willing to begin paying any amount for services.  This will typically mean that you will be responsible to pay for initial sessions with me until your deductible has been met; the deductible amount may also need to be met at the start of each calendar year.  Once we have all the information about your insurance coverage, we will discuss what we can expect to accomplish with the benefits that are available and what will happen if coverage ends before you feel ready to end your sessions.  It is important to remember that you always have the right to pay for my services yourself to avoid the problems described above, unless prohibited by my provider contract.

EAP CLIENTS ONLY

If you are using EAP there is no cost for services as EAP covers services.  Your EAP offers a set number of sessions for a particular issue.  You understand that you may request more sessions once completed, or request to be referred to an ongoing provider.

NOSHOW/CANCELLATION FEE

Cancellations and re-scheduled session will be subject to a $50 cancellation fee if NOT RECEIVED AT LEAST 24 HOURS IN ADVANCE.  Your card on file will be charged. See practice polices for more details.

METHODS OF PAYMENT

You can pay by check, cash, or credit card.  You can set up payment through Simple Practice (stripe) to pay electronically.  You can also pay through Simple Practice (stripe) as needed.  I also use Square for in-person credit card payments.  Check (made out to COLIBRI COUNSELING & CONSULTING, PLLC) and cash are also accepted.

RECORD FEE

If we need to send out notes, there is a $5 record fee per request.
